Initial Visit

We will be piloting our services with a large LTC home and performing screening to establish baseline data.

Data Interpretation

Our modalities will allow us to collect bio-metric data that would otherwise be impossible to capture without significant cost or time delays.

Care Planning

Our reports will allow a concise means for HHR to create care plans for individuals and intervene with fall intervention strategies to lower falls risk.

Care Implementation

The data will be collected by our team and will create individualized approaches to care to support LTC staff to adopt a proactive approach, ensuring the resident receives the right care, at the right time in the right place.

Quarterly Follow-Ups

Quarterly care follow-ups will ensure that KPI’s are being tracked and level of risk identified to determine if changes in the plan of care are warranted. This will also ensure the case mix index scores are reflective of care. requirements.

What is It?
A thorough Cognitive Health Assessment based on Neurophysiology is used to objectively evaluate fall risk and care needs. Initial and quarterly assessments track cognitive changes over time, ensuring continual and objective risk assessment and care monitoring.

How We Use it?
EEG, auditory, and visual recordings are digitally analyzed using specialized software for neuro-psychological and cognitive assessments, aiding in brain performance evaluation.

How Does it Help?
Values are statistically compared to normative data, generating numerical scores reflecting cognitive performance in Attention & Concentration, Information Processing, and Memory, tracking neural function changes.

Hearing Health

What is It?
Understanding age-related hearing decline is crucial for balance, social engagement, and dementia care. Healthy hearing directly impacts cognitive and social health, a well-established connection.

How We Use It?
Assessing hearing loss levels and devising care plans with personalized approaches and devices. The physiological link between hearing and balance directly reduces fall risks.

How Does it Help?
Regular hearing tests are often overlooked, but including them in screening processes enhances resident profiles. Integrating hearing health into assessments strengthens overall care, though LTCHs typically don’t offer proactive hearing assessments.

What is It?
A means of understanding how a resident interacts with their environment and how these pertain to falls risk, intervention and reduction of falls.

How We Use It?
We include this data as a further means to understanding the bio-metrics we gather and how best to implement care-planning, product support or modifications to the environment.

Knowing the full environment spectrum provides a focused intervention of falls risk including modifications to the living space.
