Our CareFall Screening Process

Gathering Usable Data to Predict & Intervene before the Fall

Initial Visit

We will be piloting our services with a large LTC home and performing screening to establish baseline data.

Data Interpretation

Our modalities will allow us to collect bio-metric data that would otherwise be impossible to capture without significant cost or time delays.

Care Planning

Our reports will allow a concise means for HHR to create care plans for individuals and intervene with fall intervention strategies to lower falls risk.

Care Implementation

The data will be collected by our team and will create individualized approaches to care to support LTC staff to adopt a proactive approach, ensuring the resident receives the right care, at the right time in the right place.

Quarterly Follow-Ups

Quarterly care follow-ups will ensure that KPI’s are being tracked and level of risk identified to determine if changes in the plan of care are warranted. This will also ensure the case mix index scores are reflective of care. requirements.

CareFall assessments

Cognitive Health

What is It?
A thorough Cognitive Health Assessment based on Neurophysiology is used to objectively evaluate fall risk and care needs. Initial and quarterly assessments track cognitive changes over time, ensuring continual and objective risk assessment and care monitoring.

How We Use it?
EEG, auditory, and visual recordings are digitally analyzed using specialized software for neuro-psychological and cognitive assessments, aiding in brain performance evaluation.

How Does it Help?
Values are statistically compared to normative data, generating numerical scores reflecting cognitive performance in Attention & Concentration, Information Processing, and Memory, tracking neural function changes.

CareFall assessments

Gait Analysis

What is It?
We employ advanced technology to prevent falls and enhance LTC residents’ quality of life. By accurately measuring gait and balance, we identify fall risk factors, track changes, and assess intervention effectiveness.

How We Use It?
It streamlines fall risk assessment, enhances profiling and stratification, boosts proactive care for high-risk individuals, reduces new fall incidences, and minimizes foot ulcer hospitalizations.

How Does it Help?
We cut operational costs by proactively identifying gait changes, streamlining data collection, analysis, and providing evidence-based care documentation, saving time and staff resources.

Phase 1: Fall Prevention Assessments

Hearing Health

What is It?
Understanding age-related hearing decline is crucial for balance, social engagement, and dementia care. Healthy hearing directly impacts cognitive and social health, a well-established connection.

How We Use It?
Assessing hearing loss levels and devising care plans with personalized approaches and devices. The physiological link between hearing and balance directly reduces fall risks.

How Does it Help?
Regular hearing tests are often overlooked, but including them in screening processes enhances resident profiles. Integrating hearing health into assessments strengthens overall care, though LTCHs typically don’t offer proactive hearing assessments.

CareFall assessments

Environmental Assessments

What is It?
A means of understanding how a resident interacts with their environment and how these pertain to falls risk, intervention and reduction of falls.

How We Use It?
We include this data as a further means to understanding the bio-metrics we gather and how best to implement care-planning, product support or modifications to the environment.

How Does it Help?
Knowing the full environment spectrum provides a focused intervention of falls risk including modifications to the living space.

Phase 1: Fall Prevention Assessments

Phase 2: Environmental Assessments

Social Connectiveness
COVID-19 highlighted the dangers of social isolation, sparking innovative projects to support older individuals living alone. Our team aims to reduce isolation and enhance communication as key performance indicators, addressing general and falls-related health risks.

Digital monitoring systems have surged in recent years, serving as effective risk-response tools. When integrated with predictive programs like ours, they become invaluable in risk prediction.

Virtual Care
As our CareFall program expands into the community we will partner with virtual care delivery partners who will be able to interpret our findings in the field and monitor care plans, ensuring the right care at the right time in the right place.

Care Mapping

Phase 3: Adding Innovation

At the heart of CareTech, we are an experienced Care Delivery company that will delivery the latest digital health solutions to care providers and their charges. We will be a portal by which developers and companies will use to deliver their technology in a bundled approach to the masses.

We have begun discussions with companies involved with vison and oral care, stroke monitoring, skin integrity , social isolation and digital monitoring. We believe that we will be the bridge the spans the gap between innovative development and implementation.

Implementation that will produce cost reductions to the healthcare system, healthier environments for care providers and better health outcomes for our seniors.
